Abstract CILJ ISTRAŽIVANJA: Glavni cilj ovog doktorskog rada bio je testiranje hipoteze po kojoj ispoljenost LEDGF regulira ispoljenost VEGF-C, što dovodi do limfangiogeneze odnosno povećane gustoće limfnih žila (GLŽ) na invazivnom rubu tumora te posljedično do progresije i metastaziranja raka pločastih stanica vrata maternice. ISPITANICE I METODE: Iz arhive Zavoda za patologiju Medicinskog fakulteta Sveučilišta u Rijeci izdvojeni su parafinski blokovi 99 bolesnica podvrgnutih radikalnoj histerektomiji i zdjeličnoj limfadenektomiji u razdoblju od 1995. do 2006. godine radi pločastog raka vrata maternice. Za procjenu imunoreaktivnosti korištena su tri primarna protutijela LEDGF, VEGF-C i D2-40. Za ispoljenost LEDGF i VEGF-C odredili smo imunohistokemijski skor. Gustoća limfnih žila analizirana je u peritumorskom, intratumorskom i van tumorskom tkivu metodom mjesta najveće gustoće (hot spot) i po mm². REZULTATI: U ispitivanoj skupini bilo je 21 (21,1%) bolesnica s pozitivnim limfnim čvorovima, a sveukupno pet-godišnje preživljenje iznosilo je 81,8%. Srednja vrijednost LEDGF H-skora, na ukupnom materijalu od 99 bolesnica, iznosila je 174,9, a VEGF-C H-skora 112,9. Nañena je značajna korelacija LEDGF i VEGF-C H-skora (p=0,03) na primarnim tumorima. Najveća gustoća limfnih žila nañena je peritumoralno, a razlike u odnosu na gustoću limfnih žila u tumoru i izvan tumora bile su statistički značajne. Invaziju limfnih prostora na tumorima prethodno obojenim s antitijelom D2-40 (LSI) imalo je 53,5% bolesnica. LEDGF H-skor je u skupini pozitivnih limfnih čvorova bio statistički značajno veći od vrijednosti za skupinu s negativnim limfnim čvorovima. Srednja vrijednost VEGF-C H-skora iznosila je 126,3 za pozitivne limfne čvorove, a 109,6 u skupini negativnih. Samo je broj limfnih žila V metodom hot spota, peritumoralno, unutar 1 mm od invazivnog ruba tumora bio značajno veći (p=0,05). Ispoljenost VEGF-C korelirala je (p=0,05) s GLŽ i statusom limfnih čvorova (p=0,03). Nismo našli korelacije ispoljenosti LEDGF s peritumorskom GLŽ ni s pojavnošću limfnih metastaza. Našli smo korelaciju LEDGF i LSI (p=0,03). U regresijskom modelu predviñanja limfnih metastaza ispoljenost VEGF-C imala vrijednost samostalnog čimbenika, uz prisutnost limfovaskularne invazije na hemalaun eozin bojanim tumorima (LVSI) i zahvaćenost parametrija. Univarijatnom i multivarijatnom statistikom (Coxova regresijska metoda) kao nezavisne čimbenike rizika za nastanak recidiva bolesti uz veličinu primarnog tumora i status limfnih čvorova, identificirali smo LSI. Takoñer, LSI se pokazala, uz veličinu primarnog tumora i status parametrija, kao nezavisni prognostički čimbenik u predviñanju preživljenja ispitivane skupine bolesnica. LEDGF i VEGF-C, peritumorska GLŽ nemaju značaj nezavisnih čimbenika u pet-godišnjem preživljenju bolesnica. ZAKLJUČCI: Pojačana ispoljenost LEDGF i VEGF- C uzrokuje povećanu peritumorsku GLŽ i pojavnost limfnih metastaza. Ispoljenost VEGF-C, u modelu multiple regresije, ima vrijednost samostalnog prognostičkog čimbenika u predviñanju nastanka limfnih metastaza, a LSI u predviñanju nastanka recidiva bolesti i preživljenja. Ispoljenost LEDGF, VEGF-C i peritumorska GLŽ nemaju značaj nezavisnih čimbenika u predviñanju recidiva i pet-godišnjeg preživljenja bolesnica.
Abstract (english) OBJECTIVES: In this study we have tested a potential role of the LEDGF–VEGF-C axis in VEGF-C tumor induced lymphangiogenesis, possibly represented by high peritumoral lymphatic vessel density (LVD), which may be one of the mechanisms leading to lymphatic space invasion (LSI) and metastatic spread of squamous cell cervical cancer. MATERIAL AND METHODS: The clinical data for this study were collected from 99 patients diagnosed with squamous cell cervical carcinoma from January 1995 to December 2005 who underwent standard surgical approach consisted of hysterectomy followed by bilateral pelvic lymphadenectomy at the Department of Obstetrics and Gynecology - Clinical Hospital Center of Rijeka, Croatia. Formalinfixed, paraffin-embedded specimens, from 99 patients were analyzed at the Department of Pathology, School of Medicine, University of Rijeka, Croatia. Tissue sections of primary tumors and its lymph node metastases were examined by immunohistochemistry for LEDGF, VEGF-C expression and LVD using three primary antibodies LEDGF, VEGF-C and D2-40. For immunoreactivity score we have used the staining intensity and the percentage of stained tumor cells. LVD was calculated as a mean value of lymphatics counted in peritumoral area. Results were assigned by using a mean value of expression and divided in two groups according to lymph node status. RESULTS: Lymph nodes metastasis were positive in 21 (21,1%) patients with squamous cell carcinoma of the uterine cervix and overall survival rate was 81,8%. VII The expression levels of LEDGF were significantly higher in the lymph node positive group (mean immunostaining scores were 190,6 and 170,7; P < 0,04). The mean immunostaining VEGF-C scores were 126,3, for lymph node positive group and 109,3 for lymph node negative group. The peritumoral LVD, within 1 mm from the tumor edge, was significantly higher between two study groups according to lymph node status. High peritumoral LVD showed significant correlation with lymph node metastasis. Lymphatic space invasion stained with D2-40 (LSI) was discovered in 53,5% patients. VEGF-C showed increased expression and correlated with high peritumoral LVD (p=0,05), and lymph node metastasis (p=0,03). LEDGF showed increased expression, but without correlation with high peritumoral LVD, or lymph nodal metastasis. In multivariate analysis by logistic regression, VEGF-C expression was independent prognostic factor for lymph node metastasis, while LSI was an independent prognostic factor for recurrent disease and survival in cervical cancer. LEDGF, VEGF-C expression and peritumoral LVD had no sigificance as an independent predictors for recurrent disease and survival in cervical cancer. CONCLUSIONS: Our findings suggest a potential role of the LEDGF–VEGF-C axis in VEGF-C tumor- induced lymphangiogenesis, represented by high peritumoral LVD, which may be one of the mechanisms leading to lymphatic invasion and metastatic spread. In multivariate analysis by logistic regression, VEGF-C expression was independent prognostic factor for lymph node metastasis while, LSI was an independent prognostic factor for recurrent disease and survival in cervical cancer. LEDGF, VEGF-C expression and peritumoral LVD had no sigificance as an independent predictors for recurrent disease and survival in cervical cancer.
