Abstract Uvod: Gubitak zuba jedan je od čestih stomatoloških problema u protetskom smislu. Uvijek se nastoji sačuvati zub koji je od velike važnosti za buduće protetsko zbrinjavanje. Važno je napomenuti da prije svake izrade intrakanalnog kolčića, a zatim i zubne krunice zub mora biti endodontski tretiran. Kod liječenja periapeksnog procesa gdje endodontska terapija nije uspjela odlučujemo se za apikotomiju tj. rezanje vrška zubnog korijena. Nakon toga zub se može protetski zbrinuti intrakanalnim kolčićem koji će osigurati retenciju buduće kunice. Danas nema dostupne literature koja nam objašnjava ponašanje apikotomiranih zuba opskrbljenih intrakanalnim kolčićem.
Cilj istraživanja: Cilj istraživanja bio je izraditi sedam 3D modela dvokorijenskog zuba. Prvi model intaktnog zuba služi kao osnova za usporedbu s drugim modelima u kojima su djelovi korijena rezani i opskrbljeni umjetnim materijalima. Na tim modelima analizirala se raspodjela naprezanja uzrokovana okluzalnim silama koje djeluju u usnoj šupljini metodom konačnih elemenata.
Materijali i metode: Prvi model je 3D model intaktnog zuba. Drugi je zub apikotomiran kosim rezom, a treći je apikotomiran ravnim rezom. Četvrti model je zub apikotomiran koso i opskrbljen lijevanom nadogradnjom, dok je peti također koso apikotomiran, ali je opskrbljen kompozitnom nadogradnjom. Šesti i sedmi model jednako su ravno rezani, ali je šesti model opskrbljen lijevanom nadogradnjom dok je sedmi model opskrbljen kompozitnom nadogradnjom. Svi modeli opterećivani su silom od 200 N.
Rezultati: Najveća naprezanja (vlačna) na intaktnom zubu javljaju se u fisurama okluzijske plohe zuba (50 MPa). Na apikotomiranom zubu, bez protetske opskrbe, osim u fisuri zuba, naprezanja (tlačna) javljaju se i na mjestu reza zubnog korijena (-46 MPa). Kod ravno apikotomiranog zuba opskrbljenog kovinskom nadogradnjom najveća naprezanja javljaju se, osim u fisurama i bukalnom korijenu (vlačna 28 MPa) i kruni zuba (tlačna), i cijelom dužinom nadogradnji u oba korijena zuba (tlačna, -24 MPa). Kod koso apikotomiranog zuba naprezanja su nešto slabijeg intenziteta. Kod apikotomiranih zuba neovisno o obliku reza opskrbljenih kompozitnim nadogradnjama naprezanja se, osim u fisurama, javljaju i u apeksnim trećinama nadogradnji (tlačna, -12 MPa).
Zaključak: Oblik rezanja korijena zuba, materijal intrakanalne protetske opskrbe kao mjesto djelovanja okluzijskih sila mogu utjecati na jačinu i raspodijelu naprezanja u promatranim površinama zubnih struktura.
Abstract (english) Introduction: Tooth loss is most common problem in prosthodontics. We allways effort to maintain the tooth for prosthetic waste. Its important to mantion that the before making post and core the tooth must be endodonticaly treated. When endodontical treatment is not adequately we decide for surgical apicotomy treatment. After that the tooth is prepared for prosthodontics work first with post and then with core. Until now is not well-know how react an apicotomised tooth with prosthodontics work.
Objectives of the research was to create seven 3D models of two-rooted tooth, first of wich being the intact tooth model that would pose as a basic for comparation to other models where parts of the tooth were cut and replaced with artifitial materials. On these models, stress distribution caused by mastication forces in the oral cavity would be analysed using the finite element method.
Materials and methods: First model was an intact tooth. Second model was apicotomised tooth with leaning cut, third model was apicotomysed tooth with plane cut. Fourth model was apicotomised tooth with leaning cut and prepared for metal endodontic post. Fifth model was apicotomised tooth, like fourth model but it was prepared for composite endodontic post. Sixth and seventh model was apicotomised tooth with plane cut, but sixth model was prepared for metal endodontic post and seventh model was prepare for composite endodontic post. All models were put under a load of 200 N.
Results: Greatest (tensile) stress in the intact tooth was noted in the fissure of occlusal surface (50 MPa). On apicotomised tooth, without prosthetic work, (compressive) stress appearance apart from fissure to location of root cut (compressive, -46 MPa). On plane cut apicotomised tooth equiped with metal post greatest (tensile) stress was noted in fissure and bucall root (28 MPa), whole tooth crown and whole lenght of post in both toot root (compressive, -24 MPa). On leaning cut apicotomised tooth lesser intensity of (compressive) stress was find out. On apicotomised tooth regardless of type cutting, equiped with composite post greatest (tensile) stress was noted in fissure and apical third of composite post in tooth root (compressive, - 12 MPa).
Conclusion: Form of cutting of tooth root, material of intraradicular prosthetic work, localisation and direction of occlusal forces could implicate and change intensity and distribution of stress on the observed tooth structures.
