Abstract Cilj: Cilj ovog istraživanja bio je istražiti da li postoje značajne razlike u seksualnom
samopoimanju bolesnika sa shizofrenijom i depresijom u odnosu na zdravu populaciju, te
ustanoviti moguće utjecaje čimbenika osobnosti na aspekte seksualnog samopoimanja i na
seksualni čin u bolesnika sa shizofrenijom i depresijom te zdravih pojedinaca. Nadalje,
ustanoviti da li postoji utjecaj psihomedikamenata i odreñenih sociodemografskih čimbenika:
spola, dobi, bračnog statusa, stupnja stručne spreme, religijske pripadnosti na aspekte
seksualnog samopoimanja i na seksualni čin.
Ispitanici i metode: Istraživanje je obuhvatilo 100 bolesnika sa shizofrenijom i 100
bolesnika s depresijom koji su medikamentozno tretirani više od deset godina, te 100 zdravih,
nasumice odabranih dobrovoljaca koji odgovaraju po spolu, dobi, bračnom statusu, stupnju
stručne spreme, religijsko pripadnosti ispitanim bolesnicima. Svi ispitanici koji su biti
uključeni u istraživanje bili su upoznati s vrstama testiranja i dali su svoju pismenu potvrdu o
informiranosti. Bolesnici sa shizofrenijom i depresijom su dijagnosticirani prema kriterijima
DSM-IV, a intenzitet shizofrenih simptoma utvrñen skorom na ocjenskom upitniku PANSS
(Pozitivna i negativna skala simptoma), dok je intenzitet depresije utvrñen ocjenskom
ljestvicom MADRS ( Mongomery-Aspergerova ljestvica za procjenu razine depresije koja se
sastoji od 10 točaka koje predstavljaju simptome depresije). Za ispitivanje sedam aspekata
seksualnog samopoimanja koristio se Bezinovićev upitnik za seksualno samopoimanje koji se
sastoji od 57 čestica koje dotiču sedam aspekata seksualnog samopoimanja ( seksualna “selfshema”,
svijest o vlastitoj seksualnosti, spremnost na seksualnu akciju, seksualni avanturizam,
negativna emocionalnost, seksualna nekompetentnost, seksualno zadovoljstvo). Za ispitivanje
seksualnog čina koristila se Ljestvica seksualnog iskustva Arizona (ASEX), za muški i ženski
spol, koji se sastoji od pet pitanja, od kojih su prva dva obvezatna, a slijedeća tri samo ako su u zadnjih mjesec dana imali seksualni odnos. Za ispitivanje seksualnih disfunkcija kao
nuspojava psihofarmaka koristila se ista ljestvica. Za ispitivanje osobina ličnosti koristio se
uputnik ličnosti IPIP ( International Personality Item Pool). Ovaj se uputnik sastoji od 44
čestice, a mjeri pet dimenzija ličnosti (ekstraverzija, neuroticizam, savjesnost, ugodnost i
otvorenost za iskustva).
Rezultati: Rezultati ukazuju da se crte ličnosti u bolesnika sa shizofrenijom i s
depresijom te zdravih ispitanika značajno razlikuju. Nadalje seksualna iskustva bolesnika sa
shizofrenijom i depresijom te zdravih ispitanika se razlikuju, te bolesnici s depresijom imaju
intenzivnije izražene sve testirane seksualne disfunkcije i od bolesnika sa shizofrenijom i
zdravih ispitanika. Seksualna iskustva značajno su povezana sa crtama ličnosti u bolesnika sa
shizofrenijom i depresijom. Tako odreñene crte ličnosti mogu predvidjeti odreñena njihova
seksualna iskustva. Nema razlika u seksualnom samopoimanju izmeñu bolesnika sa
shizofrenijom i depresijom. Zdravi ispitanici se značajno razlikuju u seksualnom
samopoimanju od bolesnika sa shizofrenijom i depresijom jer na svim skalama postižu bolje
rezultate. Crte ličnosti koje su značajno povezane sa boljim ili lošijim seksualnim
samopoimanjem bolesnika sa shizofrenijom i depresijom razlikuju se u odnosu na zdrave
ispitanike. Tako crte ličnosti postaju prediktor seksualnog samopoimanja. Seksualno iskustvo
značajno je povezano sa seksualnim samopoimanjem u bolesnika sa shizofrenijom i
depresijom i razlikuju se u odnosu na zdrave ispitanike. Tako seksualno iskustvo postaje
prediktor seksualnog samopoimanja. Antipsihotici i antidepresivi mogu značajno mijenjati
seksualno iskustvo i seksualno samopoimanje. Značajniji učinak imaju na seksualno iskustvo
nego na seksualno samopoimanje. Spolna pripadnost utječe na seksualno iskustvo i seksualno
samopoimanje. Ženski spol je prediktor s više seksualnih disfunkcija i lošijim seksualnim
samopoimanjem u bolesnika s depresijom i sa shizofrenijom te u zdravih ispitanika. Dob više
utječe na seksualno samopoimanje nego na seksualna iskustva bolesnika s depresijom ili shizofrenijom. Brak je loš prediktor za seksualno iskustvo bolesnika s depresijom i zdravih
ispitanika, ali ne u bolesnika sa shizofrenijom. Viša stručna sprema povezana je s manje
seksualnih disfunkcija i boljim seksualnim samopoimanjem u bolesnika s depresijom i
zdravih ispitanika. Broj hospitalizacija u bolesnika sa shizofrenijom i depresijom ne utječe na
seksualno iskustvo i seksualno samopoimanje. Duljina liječenja u bolesnika sa shizofrenijom i
depresijom ne utječe na seksualno iskustvo i seksualno samopoimanje. Vjerska pripadnost
utječe na seksualno iskustvo i seksualno samopoimanje u bolesnika sa shizofrenijom i zdravih
ispitanika.
Zaključak: I konačno možemo zaključiti da crte ličnosti mogu biti prediktor za
seksualno samopoimanje i seksualno iskustvo u bolesnika sa shizofrenijom i depresijom te
zdravih pojedinaca, ali konačni utjecaj crta ličnosti u bolesnika sa shizofrenijom i depresijom
te zdravih pojedinaca ovisi i o sociodemografskim čimbenicima kao i psihomedikaciji.
Abstract (english) Aim: Aim of this research was to establish if significant differences exist regarding
sexual self-perception of patients suffering from schizophrenia or depression, compared to
healthy individuals. More specific, we sought to establish possible influences of personality
factors on the aspects of sexual self-perception and sexual intercourse of patients suffering
from schizophrenia and depression, as well as healthy individuals. Furthermore, establishing
possible influence of psychopharmacs and socio-demographic factors (gender, age, marital
status, level of education, religious denomination) on the aspects of sexual self-perception and
sexual intercourse was also a part of this research.
Participants and methods: This research was conducted on 100 patients suffering
from schizophrenia and 100 patients suffering from depression, who have been treated with
psychopharmacs for at least ten years, and 100 randomly selected healthy volunteers. All
participants were informed about the various types of testing used in this research and gave
their written informed consent about participating in this research. Schizophrenia and
depression were diagnosed according to DSM-IV criteria and the intensity of schizophrenia
symptoms was established by a score on the PANSS scale (Positive and negative symptom
scale) while the intensity of depression was assessed by MADRS scale (Montgomery-Asperg
depression rating scale). Bezinović's questionnaire was used in order to assess seven aspects
of sexual self-perception. That questionnaire consists of 57 items regarding seven aspects of
sexual self-perception (sexual self-scheme, sexual consciousness, sexual readiness, sexual
adventurism, negative emotionality, sexual incompetence and sexual satisfaction). ASEX
scale (Arizona sexual experiences scale) was used to research sexual intercourse and sexual
dysfunctions caused by the use of psychopharmacs. It consists of five questions separate for
male and female gender; first two of which are obligatory and the following three apply only if participant had sexual intercourse during the month prior to completing the scale.
International Personality Item Pool (IPIP) was used to research personality traits. IPIP
consists of 44 items and measures five personality dimensions (extraversion, neuroticism,
conscientiousness, pleasantness and openness to experience).
Results: Results show that personality traits of patients suffering from schizophrenia
or depression and healthy individuals significantly differ. Furthermore, sexual experiences of
patients suffering from schizophrenia or depression and healthy individuals also significantly
differ, and patients suffering from depression have more pronounced sexual dysfunctions
compared to patients suffering from schizophrenia and healthy individuals. Among patients
suffering from schizophrenia or depression, sexual experiences are significantly correlated to
personality traits. Therefore, personality traits can predict certain sexual experiences. There
are no differences regarding sexual self-perception between patients suffering from
schizophrenia and those suffering from depression. Healthy individuals exhibit significantly
different sexual self-perception than patients suffering from schizophrenia or depression due
to the fact that on all of the observed scales they achieve significantly better results.
Personality traits, which are significantly related with better or worse sexual self-perception,
of patients suffering from schizophrenia or depression differ from those of healthy
individuals. In that manner, personality traits can become predictors of sexual self-perception.
Sexual experience is significantly related to sexual self-perception of patients suffering from
schizophrenia or depression and differs to that of healthy individuals. In that manner, sexual
experience can become predictor of sexual self-perception. Antipsychotics and
antidepressants can significantly alter sexual experience and sexual self-perception. Their
influence on sexual experience is more significant than on sexual self-perception. Gender also
influences sexual experience and sexual self-perception. Female gender is a predictor for a
higher number of sexual dysfunctions and worse sexual self-perception among patients suffering from schizophrenia or depression, as well as healthy individuals. Age has a more
pronounced effect on sexual self-perception than on sexual experiences of patients suffering
from schizophrenia or depression. Marriage is a bad predictor for sexual experiences of
patients suffering from depression and healthy individuals, but that is not the case with
patients suffering from schizophrenia. Higher level of education is related to fewer sexual
dysfunctions and better sexual self-perception of patients suffering from depression and
healthy individuals. Number of hospitalizations and length of treatment of patients suffering
from schizophrenia or depression does not influence sexual experiences and sexual selfperception.
Religious denomination has an influence on sexual experiences and sexual selfperception
of patients suffering from schizophrenia and healthy individuals.
Conclusion: We can conclude that personality traits can predict sexual self-perception
and sexual experiences of patients suffering from schizophrenia or depression, as well as
healthy individuals, but socio-demographic factors and psychopharmacs also need to be taken
into account when assessing the full influence of personality traits.
